The following resources were created by DARS and have been made available to all guardians and skilled professionals. They are not required as part of the Virginia Guardian Training.

Guardians are required to complete initial and annual reports on Form CC-1644, “Report of Guardian for an Incapacitated Person.” The form was updated on July 1, 2025 to add a statement about the Virginia Guardian Training:

Initial and annual reports must be sent the local department of social services where the incapacitated person resides along with a $5 filing fee. You can find contact information for all local departments through the Virginia Department of Social Services website:

Guardians in Virginia are considered mandated reporters. To report suspected adult abuse, neglect, or exploitation, you must contact Adult Protective Services (APS). There are three ways to make a report:

Before considering guardianship for an adult, it’s essential to explore less restrictive options to substitute decision-making. Review the new resource guide below created by the Virginia Working Interdisciplinary Network of Guardianship Stakeholders (WINGS).

Guardianship in Virginia Explained: Rights, Responsibilities, and Alternatives
Join us as we explore guardianship in Virginia. With an emphasis on person-centered care for adults subject to guardianship, we’ll learn about the critical duties and responsibilities of guardians, how to make informed decisions on behalf of another person, and how to complete the mandatory guardian training. We’ll also cover a brief introduction to the Virginia Public Guardian Program and discuss least restrictive alternatives to guardianship. A Recording of the Virginia Guardian Training
